At Brewer Physical Therapy, we accept Workers’ Compensation and most major insurance plans. Please contact your insurance company or you may call our Shreveport office at (318) 222-8892, our Bossier City office at (318) 747- 8892 or our Minden office at (318) 639-9536 to find out if we accept your insurance plan.

If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, Discover and American Express.

If you have a question related to your bill or insurance, please contact our billing specialists at (318) 222-8892.

If your insurance is an HMO/managed care plan and you are required to obtain a referral from your physician, the referral must be obtained before you can be seen for an office visit. Please coordinate this with your physician in advance. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit.

You may need a referral from your physician or medical provider (primary care doctor, nurse practitioner, orthopedic surgeon, podiatrist, chiropractor, etc.) for insurance to pay for your physical therapy treatment.

Brewer Physical Therapy will accept an initial referral by fax, by phone, or by email from your provider, and we will obtain the needed signed prescription within days of your referral, or you may bring the signed referral form with you to our office.

Telehealth Physical Therapy

Telehealth Physical Therapy

The same expert care you've come to expect from Brewer PT--provided virtually, in the comfort of your own home.  Available to patients located in Louisiana and Texas.

Q:  What does a Telehealth PT appointment consist of?

A:  Telerehab appointments are set up as a scheduled video call between the patient and physical therapist, using an encrypted, HIPAA-compliant platform that incorporates live streaming, exercise assignments, patient led manual techniques and compliance tracking.  Patients are required to have either a smart phone/ tablet or access to a computer with reliable internet.  At the scheduled appointment time, the physical therapist will initiate the call to the patient through the aforementioned app.

Q:  How long are Telehealth appointments?

A:  Approximately 45-60 minutes.

Q:  Will my virtual appointment be with my existing therapist?

A:  We will make every effort to ensure you will be scheduled with your existing therapist based on their availability.  If you are scheduled with another provider, rest assured that our physical therapists will collaborate sharing notes and case history to ensure that you receive consistent care.

Q:  Is Telehealth covered by my insurance?

A:  Insurance coverage varies by provider.  Our office will be happy to verify your insurance benefits on your behalf.
